Medical
Employee has four medical choices:
Blue Cross PPO 250
Blue Cross HSA 2100
Blue Cross HMO 100%
Kaiser Large Group
Dental
Dental insurance is provided by MetLife.
FSA, Flexible Spending Account
Beginning in 2007, FluidIQs offers a Flexible Spending Account, which provides pre-tax dollars for medical and dental costs not covered by insurance.
Copayment Schedule
FluidIQs provides 90% of each employee’s coverage premium and 50% of each employee’s dependent coverage for medical and dental. The balance of premium will be by payroll deduction.
Group Term Life Insurance and Group Long-term Disability
Group term life is $50,000 per employee. Long-term disability is based on wages. This insurance is provided by MetLife, and is covered for the employee 100% by FluidIQs.
Eligibility
New employees and their dependents are eligible for medical, dental and life insurance benefits at the first of the month after date of hire.
Vacation and Sick Pay
Vacation accrues at the rate of 40 hours every six months, 60 hours/six months after 5 years of employment, and 80 hours/six months after 15 years of service.
Sick pay is provided at 5 days per calendar year, prorated on date of hire.
Fidelity 401k
FluidIQs offers a 401k plan, with employer matching currently at 50% of employee contribution, up to 5% of annual salary. Enrollment in 401k is available on the first entry date after one year of service. Entry dates are at the beginning of each calendar quarter.
Incentive Plan
In addition to the 401k plan and employer matching, FluidIQs is finalizing the details of an incentive program which will be based on project profitability. When complete, all employees will be advised of eligibility and program specifics.
